Rules & Regulations
You must be 18 years of age or older.
– a $25 fine will be charged to your account for not cleaning up after yourself. Repeat offenders will be banned from the facility.
– (VIP Members) Bringing someone in who doesn’t have a membership or does not pay the day rate… Your account will be charged $500. Doing it more than once immediate ban from the facility.
– Stealing from the facility, Immediate ban
– No one is allowed in the tanning room but the person tanning
– If you hit the green button more than once (running the bed twice) we will be notified through our system. Not only is this illegal in the state of Pennsylvania, but it is also stealing time from our facility. You will be charged up to a $2,000 fine to your account and immediately banned from California Tan and Wellness and all associating facilities for life.
– Damaging any property of California Tan and Wellness your account will be put under review by management. If you are found to have caused the damage, one of two things may occur: an automatic ban from facility OR A fine of up to $800.
– For your health and safety you must wear protective eyewear. Failure to do so may result in permanent damage to your eyes.
Abnormal or increased sensitivity to the skin may be caused by certain medication. Consult with your Doctor before tanning. (If you are not used to UV lights it is likely you will burn your first time before your skin tans.)
To freeze or cancel membership you must come into the facility to fill out the proper forms in person. California Tan and Wellness has the right to cancel this agreement at any time for any reason. If you relocate you must show proof of relocation; IE emailing a utility bill, in order to cancel membership over the phone.
I agree and represent that all tanning, treatments, and use of all California Tan and Wellness LLC facilities shall be done at my own risk. I am physically able to undertake any and all treatments provided by California Tan and Wellness LLC or affiliated companies and/or respective agencies and employees. If I have a history of heart dieses, I will consult a doctor before becoming a member. I also understand that the legal entities that owns California Tan and Wellness LLC and all associating companies and employees will not be liable for any claims, demands for injuries, damages, action or causes of actions to me or to my property arising out of, or connected with the use of any services or facilities of such legal entities and affiliated companies and or their representative agents and employees from all such claims, damages, injuries, demands, actions or causes of action. My failure to regularly attend the facility OR the facility not having the hours I like, does not relieve me of my obligations to pay my monthly installment in full.
