You must be 18 years of age or older.
A $25 fine will be charged to your account for not cleaning up after yourself. Repeat offenders will be banned from the facility.
Stealing from the facility
Noone is allowed in the tanning room but the person tanning
Pa law requires 24 hours in between sessions. Fiesta beds are 24 hrs and S-class, Diva, and I-Bed are 48hrs in between.
Damaging any property of California Tan and Wellness your account will be put under review by management. If you are found to have caused the damage, one of two things may occur: an automatic ban from the facility OR a fine of up to $2,000.
For your health and safety, you must wear protective eyewear. Failure to use the eye protection provided to the customer by the tanning facility may result in damage to the eyes.
Overexposure to ultraviolet light causes burns
Repeat exposure may result in premature aging of the skin and skin cancer
-Abnormal skin sensitivity or burning may be caused by reactions of ultraviolet light to certain:
(i) foods
(ii) cosmetics; or
(iii) medications, including:
(A) tranquilizers
(B) Diuretics
(C) antibiotics
(D) high blood pressure medications
(E) birth control pills
Any person taking a prescription or over the counter drug should consult a physician before using a tanning device.
A person with skin that always burns easily and never tans should avoid any tanning devices
A person with a family member or past medical history of skin cancer should avoid tanning
Abnormal or increased sensitivity to the skin may be caused by certain medication. Consult with your Doctor before tanning. If you are not used to UV lights it is likely you will burn your first time before your skin tans.
The IRS requires any person(s) using indoor tanning beds to be charged an excise tax of 10%. This is out of the control of the facility. By signing this agreement, you understand, and are willing to pay that additional charge.
Disrespecting staff or equipment is terms for immediate termination. California Tan and Wellness have the right to terminate this contract at any time if they feel necessary.
To freeze or cancel membership you must come into the facility to fill out the proper forms in person. You must go through TWO billing cycles before attempting to cancel or freeze the account. If you choose to downgrade to a different membership, you must have gone through two full billing cycles of the previous membership before doing so. If you relocate you must show proof of relocation; IE emailing a utility bill, in order to cancel membership over the phone. If you attempt a charge back on any services or retail purchases, we have the right to pursue additional fees accrued during that dispute. California Tan and Wellness will also ban you from receiving future services and purchases.
I agree and represent that all tanning, treatments, and use of all California Tan and Wellness LLC facilities shall be done at my own risk. I am physically able to undertake any and all treatments provided by California Tan and Wellness LLC or affiliated companies and/or respective agencies and employees. If I have a history of heart diseases, I will consult a doctor before becoming a member. I also understand that the legal entities that owns California Tan and Wellness LLC and all associating companies and employees will not be liable for any claims, demands for injuries, damages, action or causes of actions to me or to my property arising out of, or connected with the use of any services or facilities of such legal entities and affiliated companies and or their representative agents and employees from all such claims, damages, injuries, demands, actions or causes of action.
My failure to regularly attend the facility OR the facility not having the hours I like, does not relieve me of my obligations to pay my monthly installment in full.
California Tan and Wellness have the right to cancel this agreement at any timefor any reason.
